What is the judge deciding?

Markman v. Westview Instruments assigns claim construction to the judge. Phillips v. AWH explains that the claim language is read in context from the perspective of a person skilled in the field, with the specification and prosecution history central to the inquiry. The hearing addresses meaning; it is not automatically a trial of every infringement issue.

What do the parties prepare?

Follow the assigned judge's order and local rules for proposed terms, competing meanings, supporting material, and briefs. Northern District of California Patent Local Rules 4-1 through 4-6 provide one example. Explain why a disputed meaning affects the case rather than presenting definitions with no identified consequence.

Will there be live testimony?

Check the particular procedure. Northern District of California Patent Rule 2-1(b) calls for discussion of whether the hearing will include live testimony and how to educate the court about the technology. Do not assume every court uses the same format or requires an expert witness at the hearing.
